A checklist for staff to follow when transferring patients to help them adhere to the hospital policy and maintain good standards for patient safety.
To reduce the possibility of incidents of harm, the trust has introduced the transfer checklist which contains information about the patient, to prepare the ward or department that is expecting them to maintain patient safety upon transfer.
The checklist is concise and looks at three distinct phases in the hospital setting:
- preparation for transfer (before)
- physical transport of the patient (during)
- handover of the patient to receiving staff (handover)
The trust has rolled it out across all inpatient areas and it has been positively received by staff. The checklist takes the 'Situation Background Assessment Recommendation' format which is familiar to nursing and medical staff, and ensures appropriate information is captured at the point of transfer.
