Creating driver diagrams for improvement projects


Central London Community Healthcare NHS Trust has designed this simple tool to help teams document their basic driver diagrams/action effect diagrams in a graphical format. It is useful for anyone who is looking to create or document a theory of change for an improvement project.

This tool is designed specifically for people who have a basic knowledge of using Excel. All the creation, formatting and connecting of driver boxes is automated to minimise the amount of time taken to create the driver diagram.

Creating driver diagrams (also known as action effect diagrams) is a helpful starting point for an improvement project. A well drafted diagram provides a graphical representation of a team's logic model/theory of change, describing their understanding of how the system works for a given aim.

This tool has been created to minimise the technical knowledge needed to create a visually appealing driver diagram. The basic functionality needed to modify the tool to suit your needs is described in the instructions page.
