Reviewing ‘stranded’ patients - what are patients waiting for? PDF, 69.6 KB

This rapid improvement guide sets out how stranded patients can be identified and how to capture information to address prolonged patient stays.

The aim of any review of these patients is to understand what is the plan and what is the next thing that these patients are waiting for on the day of review.

Can you please advise if technical definitions are available for the 15 stranded patient "waiting" codes, I am especially interested in code F15 "Waiting for social care reablement or home-based intermediate care time limited" which can potentially pertain to multiple services. Many thanks.
