Resources to support delivery of reducing the impact of serious infections (antimicrobial resistance and sepsis) CQUIN, parts 2c and 2d for 2018/19.

We've developed these resources alongside NHS England and Public Health England, in response to the feedback from providers and commissioners. 

Thank you to everyone who has provided feedback and supported the development and testing of these tools to ensure they are fit for purpose.

Public Health England and NHS Improvement have developed a data collection tool to facilitate the submission of part 2c review data. This data collection tool has been updated for Q3 based on user comments from Q1.

Part 2d antibiotic consumption submission tool XLSX, 80.6 KB

Use this spreadsheet to submit quarterly antibiotic consumption data for CQUIN 2d to Public Health England. Please note that we are unable to accept submissions that do not use this spreadsheet in the requested format. All data, once submitted, will be available via Public Health England AMR fingertips after an eight week data cleaning period.
