Reporting a Serious Incident to the Strategic Executive Information System (StEIS)

Reporting a Serious Incident must be done by recording the incident on the Strategic Executive Information System (StEIS). This system facilitates the reporting of Serious Incidents and the monitoring of investigations between NHS providers and commissioners.

All Serious Incidents which meet the definition of a patient safety incident should also be reported to the National Reporting and Learning System (NRLS) for national learning. Organisations with local risk management systems that link to the NRLS can report via their own systems. Organisations without this facility should report to the NRLS using the relevant NRLS e-form.

NB: StEIS and the NRLS are not currently linked and this is why separate reporting is required. Work to redevelop and combine functions of StEIS and NRLS will be undertaken as part of the development of the new Patient Safety Incident Management System.
